Dear A. Arnold Relocation Customer:

We are very sorry that you incurred damage during your recent relocation.

In accordance with the Interstate Tariff regulations, claims for loss and damage documented on the delivery paperwork must be filed in writing within nine months of the date of delivery.

Any concealed damage discovered after delivery should be filed within 90 days. The carrier must be able to inspect the damage within a reasonable amount of time from delivery for the purpose of establishing liability.

Government shipments (GSA) must file their claim in writing within 75 days of the date of delivery. Any damage on a Military shipment that you have listed on your 1840 at delivery or filed with the carrier on your 1840R within 75 days, must also be submitted to the carrier on a Statement of Claim Form within 9 months in order to receive Full Value coverage.

Therefore, please complete the following Statement of Claim Form, keeping a copy for yourself upon submitting to this office.

Incomplete information may result in undue delay in the handling of your claim. Therefore, it is to your advantage to fill out this form completely, including submission of supporting proof of value for items not repairable. An inspection or repairs may be authorized to our repair firm in your area.

Please do not secure paid estimates, repair, or dispose of any items unless cleared with this office. Salvage may be retained for items deemed non-repairable and replacement cost is paid.

We regret that the relocation of your goods did not take place without incident and apologize to you for any inconvenience.

Sincerely,

A. ARNOLD WORLD CLASS RELOCATION
